YOUR ARRIVAL
Consent for Treatment – Now that you have arrived at Perry Memorial Hospital you will be asked to sign a consent for treatment. If you are a minor your parent or guardian must sign this consent form.
YOUR ROOM
Perry is pleased to offer all private rooms to inpatients.
SMOKING
To protect your health and the health of others, Perry Memorial Hospital is a Tobacco-Free campus.
FOOD SERVICE
During your inpatient stay, every effort is made to provide you with nourishing, attractive and tasty food. You may choose from several selections for each meal within the limitations ordered by your doctor.
TELEVISION
You are provided with a color television. Please keep the volume of the television to a minimum respecting the rights of other patients.
TELEPHONE
Your room is furnished with a telephone because we realize how important it is to be able to talk with your family and friends while you are in the hospital. To make a local call, Dial 9 + 1 + the area code + the number. When making long distance calls, Dial 9 + 1 + area code + the number. You may bill your calls to your home phone, your calling card, or you may call collect. The Hospital operator may be reached by dialing 0 and local directory assistance may be reached by dialing 9 + 1 + 411. There is a courtesy phone located in the North (Medical Office Building) Lobby.
CELL PHONES
The use of two-way radios and cellular phones is permitted in most areas of the hospital. Please ask your nurse if there are any restrictions for use in your room.
PERSONAL ITEMS
Only a few personal items will be needed while you are a patient. We suggest you bring a bathrobe, bedroom slippers, nightgown or pajamas and toilet articles. We will provide everything else which may be necessary to make your stay a comfortable one.
NEWSPAPERS
You or your family may purchase a local newspaper from a vending machine in the lobby of the hospital.
PATIENT MAIL AND FLOWERS
Patient mail is delivered Monday through Saturday to the hospital and will be brought to you by one of the hospital volunteers. The volunteers will also mail your letters for you. Flowers addressed to you will be delivered to your room.
NOTARY PUBLIC
Should you require the services of a notary public, please contact the Administration Department at extension 2293 or Nursing Administration at extension at 2270.
SPECIAL SERVICES
Decoded telephones and televisions are available for the hearing impaired.
Interpreters are available for non-English speaking persons
